When you send a referral, question or job interest form, we collect the information you enter, such as names, phone numbers, email addresses, county, waiver type and the services requested. We also note which page or partner link brought you to the form.

How we protect it

• Form information is encrypted when stored and can only be opened by authorized staff with their own login.
• We never include client information in email notifications.
• Every time a record is opened, changed or exported, it is logged.
• Please do not include diagnoses or medical details in website forms. We collect those securely during intake.

How we use it

We use your information only to respond to you, provide services and coordinate with your case manager. We do not sell or share it for marketing.

Cookies and browser storage

This site uses browser storage to remember which partner link you arrived from, so a referral is credited correctly. It is kept for 60 days and is not used for advertising.
