Who we serve

Adults 18 and older, including people with physical disabilities, developmental disabilities including autism and Down syndrome, brain injury, and memory or cognitive difficulties.

Waivers we accept

  • CADI: Community Access for Disability Inclusion
  • DD: Developmental Disabilities
  • BI: Brain Injury
  • CAC: Community Alternative Care

We also accept private pay and VA benefits. Call us about other plans.

Waiver services start with a county assessment. In Hennepin County, call Front Door at 612-348-4111. Disability Hub MN can also help you understand your options at 1-866-333-2466.

Once you have a waiver and a case manager, ask them to choose Peace of Mind Care Center.
